The Infusion Providers Alliance (IPA) submitted comments to the Centers for Medicare & Medicaid Services (CMS) on the Calendar Year (CY) 2027 Medicare Physician Fee Schedule proposed rule.

Community-based infusion providers deliver high-quality, lower-cost care, yet Medicare payments for administering complex therapies continue to fall as the costs of delivering care rise. The proposed rates would further widen the payment disparity with hospital outpatient departments (HOPDs). Under the proposed 2027 rates highlighted in IPA’s comments, payments for the primary complex drug infusion code would decrease approximately 6.6 percent in physician offices and freestanding infusion clinics, while increasing approximately 9.3 percent in HOPDs.

IPA’s comments emphasized two priorities specific to the proposed rule:

  • Address the widening payment disparity. CMS should re-evaluate drug administration payments in non-hospital settings to reflect the costs of providing community-based infusion care and protect patient access to convenient, local treatment.
  • Delay proposed practice expense methodology changes. CMS should collect more comprehensive, current data and publicly disclose the effects on specific services before implementing major changes.

IPA also encouraged CMS to work with Congress to enact the Protecting Patient Access to Cancer and Complex Therapies Act (H.R. 4299). The legislation would preserve ASP-based provider reimbursement for negotiated Part B drugs, maintain beneficiary coinsurance based on the lower negotiated price, and allow Medicare to realize program savings through manufacturer rebates.

IPA warned that continued payment cuts could accelerate provider consolidation, shift patients to higher-cost settings, and threaten patient access.
